Testosterone is one of the most discussed and most misunderstood hormones in the context of health and fitness.

In conventional medicine, it's often undertreated — dismissed as a normal part of aging or addressed only when levels are severely deficient by outdated reference ranges.

Neither framing serves people well.

The research on testosterone's role in overall health is more nuanced, more significant, and more actionable than either of those framings suggests — and understanding it is worth the effort, particularly for anyone training seriously and not experiencing the results they should.

What Testosterone Actually Does in the Body

Testosterone is a steroid hormone produced primarily in the testes in men and in smaller amounts in the ovaries and adrenal glands in women.

Its functions extend well beyond muscle development and libido — the associations most people carry.

Muscle protein synthesis. Testosterone stimulates muscle protein synthesis — the process by which the body builds and repairs muscle tissue. This is the primary mechanism by which testosterone supports body composition and recovery from training. People training at high volumes with chronically low testosterone are running a significant biological headwind.

Bone density. Testosterone is a primary regulator of bone mineral density in both men and women. Low testosterone is associated with accelerated bone density loss — a significant health concern, particularly as aging progresses.

The Clinical Picture Most People Never Get

Most people who have their testosterone measured do so in a conventional primary care context — a single blood draw that measures total testosterone and compares it to a broad reference range.

This approach misses significant clinical nuance.

Total testosterone doesn't tell the full story. Free testosterone — the biologically active fraction not bound to sex hormone-binding globulin (SHBG) — is the fraction that actually exerts hormonal effects at the tissue level. A person can have total testosterone in the "normal" range while having low free testosterone due to elevated SHBG — and experience all the symptoms of testosterone deficiency.

The reference ranges used in conventional medicine are derived from broad population samples and set to identify severe deficiency. They don't identify the zone between "technically not deficient" and "actually optimal for health and performance."

Context matters. Symptoms matter. The full hormonal picture — including estradiol, SHBG, LH, FSH, and thyroid function — matters.
